My child may go on field trips with his/her registered classes
Photos of my child may be used in local media
Sharing of my child's medical information with summer school staff, bus drivers and emergency transport if needed.

I understand Summer School Fee Refunds:

Are only issued when cancellation is two weeks(2) prior to the start of the summer school class (or at least one week before a summer swim class begins). Exceptions for refunds are made for families experiencing family emergency or health/medical situations
For school age students who attend school in the Oregon School District, refunds will be transferred to their lunch account in September
For school age students who do not attend school in the Oregon School District, paper checks will be issued.

I understand and am aware that the Oregon School District does not carry insurance on program participants and I hereby waive the Oregon School District of any liability.
